There are several forms that are utilized in our department ranging from the Memorial Day Parade Application to Membership Renewals. For your convenience, they can be found here according to your inquiry.  Please follow all directions that are listed on each form for a successful submission.

Membership Application
To be filled out every year and submitted into the Recreation Center to process your Recreation Center Membership renewal or to apply for a new membership. The "Recreation Center Waiver" must accompany this form if you are applying for a new membership. Deadline for submission is no sooner than 30-days prior to your membership expiring.

Recreation Center Waiver
For those who do not have a waiver on file with us and would like to utilize the Recreation Center and/or participate in any programs in the Recreation Center. This form must accompany the "Membership Renewal" form if you are a new member.

Program Proposal Form
In order to provide the best service possible to our patrons, we require all instructor-candidates to complete a proposal form. This form is for informational purposes only and does not imply a contract between the instructor and the City of Blue Ash. This form is accepted at all times.

Adult League Registration Form (Fill In)


This is a general PDF form that you can fill in and E-Mail to the league coordinator for all of our sport team offerings. We suggest that you save it to your files for your records prior to sending. This form can also be saved and faxed to the Recreation Center.

Sports Center Use Agreement
This agreement covers the usage of the sports fields that are located at the Blue Ash Sports Center (11540 Grooms Road). There is no deadline for this form, however, the Recreation Superintendent must have already been notified prior to submitting this form.

2017 Memorial Day Parade Application (DEADLINE: May 19, 2017)


The Blue Ash Memorial Day Parade promotes a “family friendly” atmosphere that showcases elements of the Memorial Day theme which must be appropriate for all ages.  To meet this goal, we ask that all groups interested in being in the parade fill out the application in order to participate. Deadline for submission is Friday, May 19, 2017.

2016 Band/Gig Application


Band/Gig Applications are closed for 2016. Please check back in November for consideration for the 2017 season.

Food Truck Application


Food trucks that are interested in vending at select Special Events must fill this application out in its entirety in order to be considered. Those chosen to participate in our events will then be contacted in order to move forward with an official contract agreement.
