All applicants must completely fill out an application form and a background check authorization form.  Originals of both forms must be returned to the Human Resources Office at the City of Blue Ash in order to be considered for a position. The application and background check authorization form is available by clicking on Application Form on the left side menu, or copies are available from the Municipal & Safety Center at 4343 Cooper Road. The lobby is open 24 hours a day.
